In this educational video, the authors demonstrate a laparoscopic sleeve gastrectomy in a patient with situs inversus totalis, an anatomical variation with an incidence of 1:10,000 live births, with few reports in the literature of this combination: sleeve gastrectomy and situs inversus totalis. The operative steps are performed conventionally in an inverted fashion.

In this live educational video, Dr. Luiz Alfredo Vieira d'Almeida beautifully demonstrates a Roux-en-Y gastric bypass in a 58-year-old male patient using the da Vinci Xi™ robotic surgical system. The patient’s BMI decreased from 61 to 47. The dissection of the upper part of the stomach was difficult, and the expertise and know-how of Dr. Vieira d'Almeida contributed to the remarkable creation of the gastric pouch. During this procedure, a gastrojejunostomy and a jejunojejunostomy were performed manually in a stepwise fashion, which allowed for adequate procedural reproducibility. The operator also highlights the benefits of the robotic approach in a difficult case.
