This video shows the case of an 80-year-old woman admitted to our department for bile duct stone clearance after a double endoscopic failure. MRI and intraoperative cholangiography confirmed the presence of two giant (2cm) bile duct stones. A fully laparoscopic choledochotomy with stones extraction and cholecystectomy were performed. T-tube drainage was not used. The postoperative course was uneventful. The patient was discharged on postoperative day 5.
