Laparoscopy is the common approach to perform a splenectomy and frequently only requires 3 trocars. In case of splenic abscess, local inflammation can modify the anatomy and make the dissection somehow more difficult, by avoiding the identification of the usual dissection planes. This is well shown in this clinical case in which adhesions of the omentum and thickening of the peritoneum alters the view of the pedicle. Nevertheless, follow-up of basic principles allowed to complete the splenectomy using 4 trocars without bleeding.
