Laparoscopic approach is the gold standard for elective splenectomy. In the last years, single port laparoscopic surgery has offered a useful alternative, capable to furthermore reduce the aggression on the abdominal wall, but with the loss of a good exposure as major drawback. We propose the use of a mini-laparoscopic approach as another alternative. Using 3 and 5mm working ports and a 12mm port for visual control and posteriorly for the use of a vascular stapler, a correct triangulation – critical for a good exposure of anatomical landmarks - is achieved and the parietal trauma is minimized.
