With Dr. Alaa El-Ghoneimi, we have seen how redo pyeloplasties can be difficult. Transperitoneal access is required since perirenal and perijunctional adhesions prevent any satisfying access to the retroperitoneum. In some cases the ureteropelvic junction is not desirable for anatomical reasons. The ureterocalicostomy performed on the lower calyx may be the adequate solution in some rare circumstances.
