This is the case of a 65-year-old man who presented with recurrent rectal bleedings. During a diagnostic colonoscopy, a rectal polyp was excised. The pathologist reported a pT1(sm2) moderately to poorly differentiated rectal adenocarcinoma with a resection margin of less than 1mm at the base of the polyp and on the lateral side without vascular invasion. A transanal TME was decided upon in a patient with severe systemic disease. It was converted into a laparoscopically assisted procedure.
