Access to the extraperitoneal space is a key step in the totally extraperitoneal approach for the management of inguinal hernia. Different techniques have been described. However, the safest technique remains a standardized approach to the muscular sheath of rectus abdominis muscles. In this film, we demonstrate the benefit of using a port equipped with a dilatation sheath. The entire procedure is also shown.
