In this authoritative lecture, Dr. Peter Angelos briefly describes the main principles of intraoperative quick PTH assay with historical background in the context of focused parathyroidectomy. He outlines the advantages and significance of different criteria to assess the drop in the PTH assay in order to detect the multiglandular disease. He also provides an interpretation of assay and highlights the drawbacks of quick intraoperative PTH. He demonstrates intraoperative PTH levels of a case, which turns out to be asymmetric parathyroid hyperplasia.
