Résection robotisée d'un ganglion lymphatique coeliaque métastatique secondaire à un carcinome épidermoïde du poumon

In this educational video, authors present the case of a 60-year-old female patient presenting with a metastatic squamous cell carcinoma (stage cT3N2M1) of the lung diagnosed in 2018. Chemotherapy with a Pembrolizumab regimen and radiotherapy were performed. During radiological evaluation in 2020, CT-scan and MRI showed a progression towards a coeliac adenopathy (35mm). After a multidisciplinary discussion, surgical resection of the coeliac node was put forward. The coeliac region and the left side of the hepatic pedicle were exposed. Careful dissection of the metastatic node was performed, respecting the common hepatic artery and the coeliac trunk. A second lymph node was resected. The two specimens were then extracted for pathological exam. Postoperative outcomes were unevenful, and the patient was discharged on postoperative day 1. The pathological analysis confirmed the metastatic origin of the coeliac node. The lateroportal node was normal.

Colorectal cancer remains a challenging clinical entity worldwide as it affects more than one million individuals annually.

Clinical presentations vary, usually in three common ways, namely asymptomatic lesions detected during routine screening, vague clinical manifestations such as changes in bowel habits, weight loss, rectal bleeding which lead to further investigations, or emergencies with occlusion or perforation.

Cecal tumors are more difficult to detect because symptoms usually occur late and more often lead to discover an advanced disease.

In this video, authors show the case of a 65-year-old obese female patient presenting with an asymptomatic cecal cancer detected during a CT-scan performed for pulmonary embolism. A robotic approach with the da Vinci Xi™ Robotic Surgical System is demonstrated.
